Swing Bed Program

Swing Bed Program

The Swing Bed Program at NCHS is an extended care of services for those who are not yet able to care for themselves, but who no longer need acute hospital services.  Our Swing Bed Program can help make a smooth transition between acute care and your return home by providing services that range from restoring your independence to providing care and comfort at the end stages of life.  Our program includes skilled nursing care, physical rehabilitation and other services based upon your treatment plan.  If you were hospitalized elsewhere, you can transfer to the Nelson County Health System Swing Bed Program, bringing you closer to your family and home.

Who Qualifies?

Patients who have a history of at least a three day acute-stay within the last 30 days.  The patient's care provider must also determine that there is a need for additional skilled therapy.

Insurance

Medicare, Medicaid and many private insurance companies will cover swing bed care.  The swing bed coordinator will work with you to make all of the necessary arrangements and answer any of your questions.
